Job Description

Essential Functions:

  1. Receives and processes all requests for services.
    1. Completes referral form with medical and clinical information. Reviews demographics to insure accuracy
    2. Distributes referrals for assignment, scheduling, insurance verification and billing.
    3. Disseminates information and answers inquiries related to the availability of resources.
    4. Verifies physician signing Home Health Plan of Care.
  2. Establishes system for prior authorization from third-party payers.
    1. Maintains logs and files necessary to assure compliance.
    2. Develops and implements procedures for completing and filing reimbursement forms.
    3. Reviews all communication from insurance companies/other payers.
    4. Communicates with families and clients regarding the coverage for home care services.Acts as an educational resource to physicians, social workers, discharge planners, and utilization review staff regarding home health services and available coverage.
  3. Collaborates with Marketing Staff in providing education and resources to Agency staff
    and community.
    1. Educates clinical and accounting staff about insurance benefits and coverage criteria.
    2. Works with community referral sources, families, and Agency staff to problem solve and identify services available to the client either through Agency services or the community.
    3. Participates in Agency orientation for new staff in matters pertinent to client admission criteria.
    4. Provides outside facilities, if indicated, with information about availability and type of services and reimbursement.
    5. Follows up on client transition to home. Communicates with referral sources.
  4. Demonstrates teamwork and effective communication to accomplish Agency and client goals.
    1. May participate in care conferences and other team/Agency meetings.
    2. Shares client observations and pertinent information regarding reimbursement.
    3. Initiates interdisciplinary referrals and works with teams to identify and access outside resources as appropriate.
    4. Participates in Agency quality improvement activities. Initiates activities related to improvement activities in admission/intake area.
  5. Performs job in compliance with Agency policies and procedures and professional and community standards.
    1. Accepts responsibility in accordance with the role of Intake Nurse.
    2. Provides services in accordance with state and federal laws.
    3. Accepts responsibility for personal and professional development and identified learning/developmental needs.
    4. Maintains confidentiality in all aspects of the job.

Knowledge, Skills and Abilities:

Basic Requirements: 


            Associates Degree in Nursing






  • At least two (2) years nursing experience with at least one year in home care.


Current License or Certification: 



  • RN or LPN with current license to practice in state(s) of operation.
